§11(5) is that the worker truly cannot do the job they hold. The Act has no separate probation article. Practice often treats probation as employment too, so an ending still comes back to §11 or §12. This card does not decide whether the person stays.

Check when done

List the duties, and the specific events that missed them. Dates, not character.Find whether there was a period to improve, or a change of duties. If there is no record, mark “none.”Notice and severance are another card. Do not put §11 and §12 in the same sentence.

Find

If all you have is “probation failed,” do not send the letter. To walk §11 through, a lawyer should see the timeline.
